**Job Overview:**
We are seeking a dedicated Case Manager to join our team (10 hours a week.) The ideal candidate will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary services and resources to improve their quality of life.
**Responsibilities:**
- Conduct assessments to determine clients' needs and develop individualized service plans
- Provide crisis intervention and support to clients facing challenging situations
- Coordinate with external agencies and service providers to ensure clients receive comprehensive care
- Monitor client progress and adjust service plans as needed
- Offer mentoring and guidance to clients to help them achieve their goals
- Supervise and oversee case management staff to ensure effective service delivery
**Experience:**
- Previous experience in case management
- Familiarity with child welfare systems and services
- Knowledge of workforce development programs and resources
- Experience in managing crises and implementing crisis management strategies
- Ability to supervise and mentor individuals effectively
